Account Deactivation Request - Deceased or Incapacitated WayUp User

Deceased User

In the unfortunate event of the death of a WayUp user, we will work with a family member, friend, classmate, or coworker of the deceased to deactivate the account. Please use this form if you come across the profile of a colleague, classmate or loved one to request the removal of the deceased user’s account.

We will need you to submit:

  • The member's name and email address
  • The URL of their WayUp profile
  • Your relationship to them
  • Date they passed away
  • Link to obituary

We will contact you within 72 hours to notify you of the account’s deactivation or to provide additional information as needed. Be assured all information will remain confidential and after the account is deactivated all information submitted will be deleted


Incapacitated User

In the unfortunate event of a WayUp user becoming incapacitated, due to an accident, medical illness, or other cause, we will work with a person authorized to act on behalf of the incapacitated person to deactivate the account.

Please use this form to request the deactivation of an incapacitated user’s account.

We will need you to submit:

  • The member's name and email address
  • The URL of their WayUp profile
  • Your relationship to them
  • Proof of Power of Attorney

Within 72 hours after you fill out the form, we will email you to provide a copy of the incapacitated person’s ID and a copy of a Power of Attorney authorizing you to act on the user’s behalf. Be assured all information will remain confidential and after the account is deactivated all information submitted will be deleted.
